Targeted Case Management is a service provided for children ages 3 to 21 who have serious emotional disturbance, mental illness, or a substance related disorder.  The service is designed to provide linkage to needed services and coordination of multiple services or providers.  Individuals eligible must be enrolled in Medicaid, NC Health Choice or have IPRS funding.
